Job Summary
Provide assistance to the Hospitalist service in gathering and validating daily patient data, such as vital signs, ancillary reports/results, intake/output reviews and others. Performs patient rounds with the Hospitalist and independent rounds when appropriate and identifies patients to be discharged. Ensures timely execution of Hospitalist's orders. Offers input to patient care decisions and patient care planning. Act as a liaison between the physicians and the case management staff regarding issues on patient discharge planning needs. Assist in facilitating timely discharges of patients, which may include transcribing discharge orders, preparing prescriptions, and confirming discharge plans with family members/Case Management. Instruct patients/significant others and health care professionals regarding disease process, treatment plan, and admission through discharge needs. Utilizes evidence-based medicine as basis for improvements in care and outcomes of Hospitalist patients. Full time hours to include coverage for vacations, weekends, and holidays. Coordination/Facilitation Assists staff with the admission through discharge planning needs of patients/significant others. Assists in the development/review of policies and procedures. Serves as a resource to staff in practical application of policy and procedures. Acts as an advocate for patients/significant others and healthcare professionals. Assists in the prioritization of patient rounds to facilitate timely discharge of patients (i.e.: 11 am on day of discharge) and current medical needs of inpatients. Makes phone contact to primary physician/care team receiving patient to assure reception of all dictations, pertinent and pending x-ray and/or lab reports. Answer pertinent questions; tell of pending lab results that need to be followed up on. Makes phone contact to patient within 24 hours of discharge to establish success of discharge plan, review implementation of Rx, orders, treatments, follow up with primary physician, answer other questions PRN. Assists other agencies with disposition needs (death certificates, Rx clarification, insurance reviews, pt. billing concerns and other needs as they arise). Collaboration Communicate with physicians and other health care members to discuss/resolve patient care issues and to assure quality patient outcomes. Assess patients' physical and psychosocial status to include age specific needs and utilize results to plan and implement a safe, effective and appropriate plan of care interventions. Under the direction of the Hospitalist Medical Director, develop, implement, and review policies and procedures, guidelines, protocols, pathways and standing orders. Establishes ongoing relationships with service providers. Leadership Demonstrates ability to partner with Hospitalists, multidisciplinary team members, patients and families with identified issues. Demonstrates clinical capacity to listen. Demonstrates self-directed behaviors to maintain knowledge and skills. Utilizes current research, literature and technology to demonstrate clinical expertise. Understands the principles of self-determination and informed consent and, assists in the education of patient/family in the process of decision-making. Understands and responds to the changing health care environment and how actions impact financial viability of the department and hospital. Participates in professional organizations and affiliations related to role.

Education/Experience/Certifications:
ADVANCED NURSE PRACTITIONER
* Registered nurse in the State of Wisconsin.
* Graduate of a master's level nurse practitioner program.
* Certification through ANCC or ANA.
* Certified in the State of Wisconsin as an Advanced Practice Nurse Prescriber or certification eligible, obtaining
certification at the first opportunity.
* Maintains current CPR certification, ACLS certification desirable.
* Experience requirements may vary.
PHYSICIAN ASSISTANT
* Graduation from an accredited physician assistant program.
* Licensed by the State of Wisconsin.
* Certification from the National Commission on Certification of Physician Assistants.
* Maintains current CPR certification, ACLS certification desirable.
* Experience requirements may vary.
