This position offers a flexible schedule designed to support work-life balance. Providers can arrange their hours between 8:00 AM and 8:00 PM, seven days a week, allowing for flexibility in managing their workload. No on-call responsibilities, weekend shifts, or holiday coverage are required.

HouseCalls is a unique, quality-focused program that delivers care directly to patients where they live. As a Nurse Practitioner or Physician Assistant, you will perform annual in-home health evaluations for Medicare Advantage and other health plan members.

This is a field-based, non-prescribing position focused on preventive care, identifying health risks, addressing gaps in care, and helping patients connect with appropriate resources. You will also partner closely with primary care providers to ensure continuity and coordination of care.

• Perform comprehensive in-home health assessments, including:
  • Reviewing medical history and medications
  • Conducting physical examinations and obtaining vital signs
  • Completing evidence-based screenings and point-of-care testing when applicable
• Identify and accurately document diagnoses to support care planning and risk adjustment efforts
• Communicate assessment findings and recommendations to primary care providers
• Recognize urgent or emergent health concerns and take appropriate action
• Educate members about health conditions, medications, and treatment adherence
• Assess social and environmental factors that may impact health and connect members to community resources when appropriate

Nurse Practitioners

• Current ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care

Physician Assistants

• NCCPA certification and active state licensure

For All Candidates

• Active, unrestricted state license (or ability to obtain before start date) with no current disciplinary actions or restrictions
• Valid driver's license and reliable transportation for conducting home visits
• Ability to perform assessments in a variety of home settings, including standing, sitting, bending, and kneeling as needed
• If practicing in a state that allows independent NP/PA practice, candidates must obtain applicable licensing board approval when required. Eligible providers must apply within required onboarding timelines and maintain compliance with state regulations.

• One or more years of clinical experience, ideally in family practice, geriatrics, home health, or a related setting
• Experience using electronic medical records and healthcare technology platforms
• Ability to safely transport clinical equipment weighing up to 30 pounds and navigate stairs during home visits
• Strong communication and relationship-building skills with Medicare-eligible and geriatric populations
