The Registered Dietitian will provide telephonic education and support to members with chronic illness. The Registered Dietitian will be responsible for improving health outcomes of and promoting appropriate preventive care for members with certain scenarios such as Diabetes, Asthma, COPD, CAD, GI, Oncology and other medical conditions needing nutrition education and support. This role will also include communication with health care professionals to ensure goals of a member's treatment plan are met, as well as ensuring that members are linked with the appropriate resources and are adopting healthy lifestyle practices to reduce risk factors. The Registered Dietitian will identify participant's behaviors and provide customized health coaching to individuals based on their health and nutritional needs.

What You Will Be Doing

  • Provides nutrition education and wellness coaching to help promote the member's ability to self-manage.
  • Conducts assessments, surveys and develops a education plan and delivers health education to members.
  • Create a personalized action plan that will will include short and long term goals prioritized by the member, specific activities/interventions to achieve goals.
  • Assists the member and caregiver to understand their illness, make life style changes, adhere to medication regimen and work with their provider to develop a symptom response plan. Collaborate with multidisciplinary teams.
  • Identify and address barriers that are preventing the member from meeting their goals.
  • Counsel individuals on daily nutrition and medical nutrition therapy, including reinforcement of physician/nutritionist developed custom diet plans
  • Provide guidance and direction on accessing community based services for home modifications, transportation, meal preparation etc
  • Focus on prevention, health promotion, maintenance, restoration and education.
  • Develops strong relationships based upon trust with individual participants.
  • Assists clients in identifying, prioritizing, and implementing choices. Empowers clients to reach their goals, through direction, encouragement, accountability and counseling.
  • Provides advice and instruction for fitness, good nutrition, and management of weight, stress, and health into a participant's lifestyle
  • Commits to continued learning in all areas relevant to the position. Maintain and develop knowledge about various health topics and behavior change strategies.
  • Utilizes established workflows and collaborates with internal & external stakeholders.
  • Complies with all policies and procedures to ensure that the program's outcomes measurements meet HEDIS and NCQA standards.

What We Require

  • 5+ years related work experience in working with acute and chronic populations to provide nutritional/health education, self-management, wellness and support in related topics

  • Related Bachelor's degree required Food and Nutrition, Dietetics or Health Education is preferred.

  • Required Registered Dietitian Certification Upon Hire

  • Experience working in out-patient clinical setting or doing predominantly telephonic nutrition health education and coaching is highly preferred

  • Experience in health behavior change/health coaching/health education a plus

  • Strong interpersonal communication skills and ability to effectively interact with persons from a variety of backgrounds and experiences in a professional, diplomatic, and courteous manner.

  • Excellent analytical/critical thinking skills, including the ability to identify problems, research and analyze issues from different perspectives, organize information, reach sound conclusions, and make appropriate decisions

  • Able to work with protected health information and maintain absolute confidentiality and privacy in accordance with HIPAA regulations

  • Excellent computer skills including working knowledge of Microsoft Office, Word and Excel and the internet.

  • Highly organized and proficient individual that can work quickly and with little direction A "team" player able to work well in an energetic, dynamic and deadline oriented environment

General Physical Demands

Sedentary work: Exerting up to 10 pounds of force occasionally to move objects. Jobs are sedentary if traversing activities are required only occasionally.
Must be able to travel occasionally to multiple locations for work (i.e. travel to attend meetings, events, conferences).

Typical Annualize Offer/ Hiring Range: $65,400 - $81,700

Annualized Salary Range: $65,400 - $106,200

Final pay will be determined with consideration of market competitiveness, internal equity, and the job-related knowledge, skills, training, and experience you bring.
