Family Educator/Home Visitor/Case Manager

The Champaign-Urbana Public Health District (CUPHD) ishiringa full-time Family Educator/Home Visitor/Case Manager position with their GREAT Start Prevention Initiative Birth to Three Home Visiting Program. The GREAT Start program provides free and voluntary home visiting services and support to families in Champaign County with children under the age of three, and prenatal families.The selected candidate will perform professional work in the Baby TALK Model of family engagement.

Responsibilities include:


  • Conducting child developmental assessments.



  • Facilitating developmental activities with children and caregivers, developing family goal plans, and providing case management and connection to community resources.



  • Programgoalsinclude supporting the child's development through strengthening the parent-child relationship,establishingand deepening working relationships with families, empowering parents to set goals for themselves and their children, and supporting families' mastery in their growth and development.


Requirements & Qualifications:


  • Graduation from an accredited college or university with abachelor'sdegree in sociology, social work, psychology, human services,childdevelopmentor a closely related field,



  • Avalid driver's license, dependable transportation,and proof of auto insurance.



  • Spanish speaking individuals are highly desirable.



  • Experience working with parents and children under three years of age is highly preferred.


CUPHD provides a comprehensive benefits package which includes:


  • CUPHD offers qualified bilingual employees anadditional$3.00 per hour, upon approval.




    • Eligible employees may complete languageproficiencytesting upon hire and can begin receiving the stipend as soon as they are approved.




  • Health, dental, and vision insurance with enrollment beginning on the employee's first day of employment



  • 14paid holidays per year



  • 12 sick daysearnedper year



  • 15 days of paid time offearnedin the first year of employment

The expected annual starting salary is $52,556. Full annual salary range for a Class 6 Case Manageris $52,556 - $70,950 for a 40-hour week.
