Position Summary: The Social Services Coordinator is responsible for linking the elderly, especially those who are frail and disabled, or the disabled residents of the project to the supportive services they need to continue living independently. Service coordination means the activity of linking a project resident to needed supportive services or medical agencies in the general community. Additionally, the term may cover case management, both formal and informal, in which the service coordinator assesses service needs, determines eligibility for public services, and makes resource allocation decisions. Essential Job Functions:
- Provides general case management (including intake) and referral services to all residents needing suchassistance.
- Mayprovide formal case management (i.e., evaluation ofhealth,psychological and social needs, development of an individually tailoredcase plan for services and periodic reassessment of the resident'ssituation and needs) for a resident when such services are not available through the general community.
- Establishes linkages withappropriate agenciesand service providers in the community; shops around to determine/develop the best "deals" in service pricing, to assure individualized, flexible, and creative services for the involved resident(s).
- Sets up a directory of providers for use by both project staff and residents.
- Refers and links the residents of the project to service providers in the general community.There are, for example, case management, personalassistance,homemaker, meals-on-wheels, transportation, counseling, occasional visitingnurse, preventive health screening/wellness, and legal advocacy.
- Educates residents on service availability, application procedures, client rights, etc., providing advocacy asappropriate.
- Monitors the ongoing provision of services from community agencies and keeps the case management and provider agency current with the progress of the individual.Manages the provision of supportive services where applicable.
- Helps the residents build informal support networks with other residents,familyand friends.
- May set up volunteer support programs with service organizations in the community.
- May provide training to project residents in the obligation of tenancy or coordinate such training.
- May educate other staff on the management team on issues related to aging in place and service coordination, to help them to better work with andassistthe residents.
- May develop case plans in coordination with community assessment services or with a PAC (Professional Assessment Committee).
- Maintainsaccuraterecords and files inmostconfidential manner.This filewill be locked and reviewed only by theExecutive Director, Executive Director or CEO ofthe company, whenappropriateand/or needed.
- Completesother duties as assigned by theExecutive Directorbutcannotbe assigned the following duties: providing social activities; assessing residents' level of functioning; providing transportation; providing medical services or advice; providing direct services or care; or any office/clerical tasks.

Starting pay ranges from $22.90 - $30.30 per hour. Factors such as scope, responsibilities, experience, education, job-related skills, internal equity, and market considerations may influence base pay. Qualifications: Education:
- A Bachelor of Social Work or degree in Gerontology, Psychology, or Counseling is preferable; a college degree is fully acceptable.However, individuals without a degree, but withappropriate workexperience, may be hired.
Training and Experience:
- Twoyears of experiencewith progressive responsibility in management functions.
Knowledge and Abilities:
- Demonstrated workingknowledge of supportive services and other resources for senior citizens and non-elderly disabled in the area served by the project.
- Demonstrated ability to advocate, organize, and problem-solve with proven results for the elderly and disabled served.
