Hospice Nurse Practitioner Lead - Part Time 48 hrs bi-weekly - weekdays 8am-4:30pm occasional on-call rotation. The Nurse Practitioner Lead position is responsible for providing compassionate, patient-centered care to individuals with life-limiting illnesses, emphasizing comfort, dignity, and quality of life. This position is responsible for conducting comprehensive assessments, managing individualized care plans, and prescribing treatments to control pain and other symptoms in collaboration with the hospice medical director and interdisciplinary team. This position is also responsible for serving as a clinical resource for patients, families, and team members, offering education and support while ensuring care meets regulatory, ethical, and professional standards. Complies with all state and federal laws and regulations related to hospice care in addition to Ecumen hospice program's policies and procedures and supports their continuous evaluation and improvement. Participates in the hospice program's quality assessment and performance improvement program. 2. Responds to urgent patient needs and provides after-hour coverage as scheduled 3. Manages symptoms of terminal illnesses by developing an individualized care plan, prescribe medications/treatments, and obtain consultations and appropriate referrals. 4. Provides clinical information and makes recommendations to the hospice medical director for interpretation for eligibility for hospice recertification. 5. Serves as the attending provider for hospice patients when selected. 6. Assesses hospice patients by reviewing their clinical information and provides documentation if the patient's anticipated life expectancy is 6 months or less if the illness runs its normal course. 7. Prepares, in consultation with the patient's attending physician, if any, and the hospice IDG, and patient or representative, an individualized written plan of care and services for each patient that reflects patient and family goals and interventions based on the needs identified. 8. Serves as the primary point of contact and lead resource for the Nurse Practitioner team. Supports onboarding and training of new team members. 9. Develops and delivers clinical education to Ecumen Hospice Team Members and community partners. Driving Requirements: Local Travel Required Nurse Practitioner Lead HCBS Minimum required qualifications: * Graduate from an accredited School of Nursing and accredited Nurse Practitioner Program. * Must be licensed within the state in which working and must maintain licensure. * Accreditation from an approved certifying body for advanced practice nursing. * 1 year of related experience * Ability to communicate effectively in both written and verbal formats. * Proficient with Microsoft Office Suite * Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident's Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations. Preferred qualifications: * Prior experience working in Hospice Licenses & Certifications
