ABOUT THE HUMAN SERVICES DEPARTMENT AT JEFFERSON COUNTY

Jefferson County Human Services developed the Community Support Program in 1996 to focus on helping consumers remain in the community who have a severe and persistent mental illness. CSP assists people in continuing their own self-developed recovery journey by offering supports for school and employment, mental and physical health needs, symptom management, medication monitoring, therapy, substance use treatment, and psychosocial rehabilitation to coach and develop skills in daily living tasks.

WHAT YOU WILL DO

The purpose of this position is to provide a variety of critical roles in collaboration with the psychiatric care provider. This position will play a key role in both direct service and staff education.

SOME ESSENTIAL RESPONSIBILITIES

You will be successful in this role by completing the following tasks and responsibilities:


  • Manage the medication system, administer injectable medications, and document medication treatment.
  • Screen and monitor clients for medical problems/side effects.
  • Communicate and coordinate services with other medical providers.
  • Engage in health promotion, prevention, and education activities with consumers in the office and the community. (i.e. assess for risky behaviors and attempt behavior change)
  • Perform labs and monitoring of health conditions as needed.
  • Educate other team members to help them monitor psychiatric symptoms, physical health issues, and medication side effects.
  • When clients agree, develop strategies to maximize the taking of medications as prescribed. (i.e. behavioral tailoring, development of individual cues and reminders)
  • Attend and participate in team meetings.
