Job Title: Clinical Social Worker

Overview: Join our dynamic healthcare team as a Clinical Social Worker, where you'll play a critical role in supporting patients and their families through the complexities of healthcare. Your expertise will help identify psychosocial factors that influence medical treatment and discharge planning, ultimately enhancing patient outcomes. We are looking for a compassionate and skilled individual to provide essential psychosocial care in alignment with JHACH policies and procedures.

Key Responsibilities:

• Conduct comprehensive psychosocial assessments to understand and address patients' and families' emotional and social needs that may impact medical treatment and recovery.
• Perform suicide risk assessments and, if necessary, facilitate the Baker Act process for patients, providing support for placement in psychiatric facilities.
• Serve as a key liaison, coordinating care by referring patients to multidisciplinary team members and connecting families to community resources.
• Deliver a range of therapeutic interventions, including crisis intervention, grief counseling, problem-solving support, and psychoeducation to promote healthy coping strategies.
• Collaborate closely with the medical team to develop comprehensive care plans, identify psychosocial barriers to discharge, and ensure smooth transitions.
• Maintain accurate and timely electronic psychosocial documentation and record keeping, aligning with best practices and regulatory standards.
• Actively participate in team meetings, huddles, and rounds, keeping communication open with colleagues regarding patient needs and treatment interventions.

Qualifications:

• Master's degree in Social Work from an accredited program.
• Minimum of 2 years of clinical social work experience.
• Current Licensed Clinical Social Worker (LCSW) in Florida.
• Exceptional listening and communication skills, both oral and written.

Salary Range: Minimum /hour - Maximum /hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
