Group Health Cooperative of South Central Wisconsin. BETTER TOGETHER This is a full-time hybrid role (40 hours per week). Candidates must be residents of Wisconsin. Under the direction, guidance and general supervision of the Population Health Manager, the Population Health Specialist plays a crucial role in coordinating and supporting GHC-SCW's population initiatives, community partnerships, community health programs, and social work services. They actively contribute to various community health programs, including the Bridges to Access Program, Stay Healthy Visit Program, Refugee Assistance Program, Primary Access for Kids Program, and Financial Assistance Program. In addition to program involvement, the Population Health Specialist aids in the development, monitoring and sustainment of partnerships and charitable contributions. The position actively supports the organization's health equity initiatives for both members and patients. This position is also a key contact for GHC-SCW clinic staff that assist in identifying and directing patients to other community services and resources that may be available.
