As a Licensed Practical Nurse in our ambulatory care setting, you will play a vital role in delivering high quality patient-centered care under the guidance and supervision of the Manager and Provider team. This role is perfect for a motivated, compassionate nurse who enjoys teamwork, organization, and making a real impact on patient outcomes every day.

In this role, you will provide hands-on nursing care, collect and assess patient data, administer medications and treatments, and ensure accurate documentation of care while supporting provider documentation. You will help drive clinical excellence across our service line, always practicing within the scope of the Missouri State Board of Nurse Practice Act and maintaining the highest standards of care.

What You'll Do:

  • You will accurately document all patient care activities in the electronic medical record for accountability and reporting.

  • You will collaborate closely with the care team to ensure seamless, high quality patient care.

  • You will collect patient data and make clinical observations that support assessments and care planning.

  • You will anticipate provider and patient needs through skilled nursing assessment.

  • You will perform pre-visit screenings for assigned patient populations.

  • You will assist with implementing care plan interventions and support patient and family education.

  • You will participate in quality improvement initiatives focused on outcomes, safety, and compliance.

  • You will maintain clinical competency, credentials, and current nursing standards of practice.

  • You will provide respectful, culturally competent care that honors patient rights and choices.

  • You will support a safe, inclusive environment for patients, families, and staff.

  • You will deliver age-appropriate care meeting physical, psychosocial, educational, and safety needs.

  • You will represent the organization with professionalism, courtesy, and compassion.

  • You will maintain compliance with statutes, regulations, standards, the Corporate Compliance Plan, and Code of Conduct, and participate in required training and compliance activities.

What You Need to Bring (Minimum Requirements)

  • Graduation from an accredited practical or vocational nursing program

  • Licensed as a practical nurse in the State of Missouri

  • Three years experience in an LPN clinical service setting

  • Experience providing operational leadership and direction to staff

  • Certified BCLS

  • Preferred Qualifications
