The Fetal Concerns Care Coordinator (FCRNCC) provides consistent, collaborative, and quality care to promote optimal clinical outcomes for patients identified with fetal concerns during the prenatal course of their pregnancy. The FCRNCC supports the successful transitions of care by ensuring the appropriate communication with members of the interdisciplinary team, including the referring providers, outpatient and inpatient providers, and their staff.
The FCRNCC position requires strong leadership, communication and organization skills; tact and sensitivity to patient/family needs and issues; commitment to professional standards of conduct, practice, and ethics; and the ability to work well with other health care professionals in a multi-disciplinary model. The FCRNCC role requires the ability to meet deadlines in an efficient manner during changing priorities. The FCRNCC seeks opportunities to improve and enhance one's knowledge of discharge planning. The FCRNCC needs to be self-directed, with sensitivity to key customer priorities, adjusting work environment accordingly.
