Position Summary:
The Center for Fibroid Care Clinical Care coordinator plays a crucial role in managing clinical triaging, coordinating services between the Fibroid Center providers including Minimally Invasive Gynecologic Surgery (MIGS) surgeons, interventional radiology, nutrition, hematology and other relevant departments. The care coordinator ensures seamless patient care and efficient operations, especially as we expand our services to various geographical locations for patients who suffer from fibroids. The coordinator plays a vital role as a leader in the center functions.
Job Responsibilities:
- Assess and prioritize patients according to clinical acuity to ensure timely and appropriate care. Collect reported symptoms, pertinent clinical information, and recent imaging reports; distribute required patient surveys; compile completed survey responses and supporting documentation into a comprehensive summary for NP/MD review to support initial triage and consultation.
- Support the center director in daily functions to optimize patient care.
- Pend orders for imaging to NP as needed for initial consult, assist patient in scheduling required imaging and ensure completion prior to visit.
- Collaborate with the Fibroid Center care team to establish effective communication and coordination channels. Facilitate regular meetings to discuss patient cases, treatment plans, and progress.
- Serve as the primary point of contact for patients and their families throughout their treatment journey. Ensure seamless coordination of appointments, consultations, diagnostics, and procedures, aiming for optimal patient experience and outcomes.
- Assist with data organization and collection for Fibroid Center research studies; support coordination of ongoing research initiatives.
- Coordinate and schedule Fibroid Center meetings, including preparation of agendas and presentation decks.
- Support the planning and execution of Fibroid Center events by coordinating patient invitations, developing presentation slides, securing event locations, and managing related logistics.
- Assist with the creation of promotional and educational materials, including flyers and information booklets, while collaborating with marketing and social media teams to increase visibility of the Fibroid Center and highlight program accomplishments and milestones.
- Provide periodic check-ins with patients via telephone at set intervals as designated by provider (ex: every 3 months, 6 months, etc), coordinate any necessary follow-up visits.
- Provide comprehensive information to patients about fibroids, available treatment options, and any pre- or post-operative care requirements. Provide appropriate education and counseling related to various procedures offered at the fibroid center
- Monitor patient outcomes, treatment compliance, and satisfaction levels. Identify areas for improvement and actively participate in quality assurance and performance improvement initiatives.
- Maintain accurate and up-to-date patient records using the electronic health record system. Ensure that all relevant information is documented appropriately and securely.
- Stay updated with the latest advancements in fibroid care, medical treatments, and healthcare coordination practices. Participate in workshops, conferences, and training programs to enhance professional knowledge and skills.
- Support the management team in planning and implementing the Center's expansion to new geographical locations. Assist in setting up new facilities, liaise with local healthcare partners and ensure a smooth transition for patients and staff.
Minimum Qualifications:
To qualify you must have a Valid and active Registered Nurse (RN) license in the state of practice.
Bachelor of Science in Nursing (BSN) degree preferred.
3 years of experience required.
Preferred Qualifications:
Proven experience as a Care Coordinator or in a similar care management role.
In-depth knowledge of fibroids, gynecological conditions, and various treatment modalities.
Excellent communication, interpersonal, and organizational skills.
Strong analytical and problem-solving abilities to handle complex patient cases effectively.
Ability to work collaboratively in a multidisciplinary team and establish positive relationships with patients and healthcare professionals.
Familiarity with electronic health record systems and proficiency in using healthcare software and technology.
Commitment to maintaining patient confidentiality and adhering to ethical standards.
Qualified candidates must be able to effectively communicate with all levels of the organization.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $127,500.00 - $133,960.71 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
