Requirements

The ideal candidate will possess the following qualifications:



  • Valid PA driver's license with clean motor vehicle report.
  • Basic computer skills and experience with an EMR system preferred.
  • Case Management experience with adults and children preferred.


Educational Requirements:



  • Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education; or
  • Registered nurse; or
  • A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years of experience in direct contact with mental health consumers; or
  • A high school diploma and five years of mental health direct care experience in public or private human services with employment as a case management staff person prior to April 1, 1989.

We are seeking a community-based Recovery Coach to join our team in Colmar, PA serving clients in Montgomery County.



Recovery Support

  • Assist individuals in creating and following personalized recovery plans.
  • Coach on skills, coping strategies, goalsetting, and overcoming barriers.
  • Provide encouragement, mentoring, and ongoing support.


Community Integration

  • Help individuals engage in community activities and use community resources.
  • Build community partnerships and support independent participation.
  • Address challenges related to stigma.


Advocacy & Relationships

  • Build trusting, hopefocused relationships.
  • Advocate during team meetings and coordinate with clinical and family supports.
  • Promote peer involvement and support networks.


Program & Administrative Duties

  • Maintain required documentation and meet contact and productivity standards.
  • Participate in oncall rotation, meetings, trainings, and supervision.
  • Provide flexible, community-based services, including evenings/weekends.
  • Perform additional duties as assigned.


Earn $22.29 per hour


Additional $2 per hour available with the selection of our Enhanced Pay option!
