The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience. Job Overview: The Social Worker provides direct clinical social work services to patients and their families in an interdisciplinary team approach. Qualifications:
- Graduation from an accredited graduate school of social work with an MSW, master's in social work degree.
- LSW or LCSW licensure is required by the NJ State board of social work examiners.
- Three or more years experience in a hospital setting preferred.

Essential Functions:
- Completing initial psycho-social assessments on assigned patients, providing psychotherapeutic counseling with individuals, family or groups as needed, giving clinical consultations as needed, providing patient-centered advocacy, collaborating with interdisciplinary team, managing complex discharge plans, building and maintaining relationships with internal/external resources, making referrals to community agencies and entitlement programs and continuing ongoing education,
- Ensure continuity of care for patients with complex post-acute needs and maximize the patients' and families abilities to meet the continuing needs within their social and financial framework,
- Understands all stages of growth and development to assess each patient's age-specific needs to provide individual treatment and care,
- Accurately documents in EMR ongoing social work services and reassessment reflecting the patient's level of functioning, identification of support networks, development of problems or needs and discharge planning,
- Assists with initial assessment on bio-ethics case consultation and ongoing end-of-life intervention as necessary.
- Attends educational activities to keep current with social work practice and to maintain required state licensure or certification,
- Consults on a regular basis with the interdisciplinary team concerning social work intervention and attends interdisciplinary team rounds to provide feedback on social work issues and activities to help ensure a safe and timely discharge,
- Demonstrates a working knowledge of crisis intervention theory and effectively assists the patient/family in defining the crisis situation and establishing a sense of temporary equilibrium.
- Demonstrates a working knowledge of medical terminology, acute and chronic illnesses and related processes, and understands the impact on the patients' biopsychosocial systems, length of stay and individual care.
- Demonstrates a working theoretical knowledge of human behavior and develops an appropriate social work intervention plan that addresses the patient's age-specific needs,
- Demonstrates skills as a patient-centered advocate, change agent, facilitator, mediator and problem solver depending on the patient/family needs,
- Identifies and collaborates with legal staff, Director of Case Management and Risk Management on issues of: incompetence, guardianship, abuse, neglect and guarantor decisions,
- Identifies potential barriers to discharge and collaborates with Supervisor and interdisciplinary team members to resolve in a timely manner.
- Interviews patients, family members and significant others to obtain relevant information for the completion of a psychosocial assessment and Social Work treatment plan,
- Maintains current knowledge of National Patient Safety Goals, supports a culture of safety and provides a secondary role in ensuring core quality indicators are met.
- Participates in patient conferences, educational conferences, hospital and community committees or task forces, performance improvement teams as assigned,
- Provides information to patients, families and interdisciplinary team members regarding available community resources and other post-hospital referrals in accordance with patient choice needs.
- Responds to referral sources in a timely manner,
- Provides psychotherapeutic counseling to individuals, family and groups, as needed, to enhance coping skills with the patient's family's reaction to the onset of or ongoing adjustment to a chronic illness or injury, post-hospital care needs, end of life issues, and lifestyle changes.
