-Demonstrates advanced skills in administering physical therapy procedures.
-Assesses patient response to treatment, modifying when appropriate with input from the supervising therapist.
-Demonstrates awareness and use of proper safety measures regarding patients, others in the treatment area, and himself.
-Interacts regularly with others involved in the patient's care to incorporate an interdisciplinary treatment approach for quality, coordinated care.
Complies with departmental policies for documentation. 10%
-Documents all aspects of assessment, planning, treatment, and discharge in the medical record (and in the working PT chart) in a timely, legible manner.
-Long-term and short-term goals are measurable and achievable within a specified amount of time.
-Daily treatment records are noted in the working PT chart on a daily basis and are clear and professional in their terminology.
-Weekly patient conference notes are succinct and contain information regarding progress to date and focus of further treatment.
-Interdisciplinary meetings are documented by the Primary Therapist in a timely manner.
-Obtains necessary evaluations and re-evaluations in a timely manner.
Participates in patient discharge preparations. 10%
-Provides Case Managers with appropriate DME referrals in an adequate amount of time in which the Case Manager is able to secure the equipment for the patient prior to D/C from RCP.
-Provides information regarding follow-up services which may include a referral to other community resources.
Actively participates in patient and family education process. 10%
-Provides education and training to patients, families, and caregivers regarding diagnoses, progress, long-term functional outcomes.
-Completes patient/family education in the functional areas noted on the Patient/Family Education Sheet prior to a TLOA or D/C (whichever is appropriate).
-Supports and conducts HILS as appropriate.
-Supports and provides educational material to patients/families for support groups as well as community resources.
Productivity 5%
-Strives to meet department productivity standards.
-Assists with coverage on other units when caseload permits and when multiple caseloads may be modified easily to permit one person to assist with coverage in another area.
-Complies with established standards of efficient patient scheduling to maintain appropriate levels of productivity when covering the inpatient unit
Provides oversight to PTA students and Technical Assistants as assigned. 5%
-Exercises sound judgment in delegating tasks to the Technical Assistant.
-Completes an orientation to the TBI/YCVA Program for the PTA student.
-Coordinates with appropriate people for an effective learning experience for the student.
-Provides timely positive and constructive feedback to the Technical Assistants and students.
-Completes mid-term and final evaluations in a timely and appropriate manner for the student
Hospital, Department, and Team Responsibilities 5%
-Attends staff meetings,team meetings, and mandatory in services as scheduled.
-Completes mandatory hospital training annually and in a timely manner.
-Completes TB test procedures by the end of the employee's birth month.
-Completes assignments in a timely and thorough manner
-Assists with care and maintenance of departmental equipment and supplies.
-May be requested to help with routine clean up duties at day's end as needed.
-Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
