Responsible for collaborating with the healthcare team to assess patient needs, develop care plans, and connect individuals with appropriate community resources. This role provides counseling, advocacy, and coordination of services to improve the overall well-being of patients and promote successful healthcare outcomes.
Does this position require Patient Care? Yes Essential Functions -Conduct comprehensive assessments of patients' social, emotional, and practical needs to identify areas requiring intervention or support. -Collaborate with the healthcare team to develop individualized care plans that address patients' psychosocial concerns and align with their healthcare goals. -Assist in identifying and addressing barriers to care, such as financial limitations, transportation issues, or lack of social support. -Provide counseling and emotional support to patients, families, and caregivers dealing with healthcare-related challenges, such as chronic illness, terminal conditions, or caregiving stress. -Offer active listening, empathy, and guidance to help individuals cope with their emotions, make informed decisions, and enhance their overall well-being.
