As a Homecare/ Hospice Social Worker, you will: * Provide supportive services to help meet the patient/family social, emotional, economic, and environment needs in relation to the impact of a chronic, acute, or terminal illness though education, support/counseling, and referrals/coordination of community resources. * Complete timely visits to meet patient/family needs and ensure program compliance. * Collaborate with the interdisciplinary team to provide whole person care addressing bio-psycho-social needs of patients and family. * Provide patients/families with the psychosocial support needed to cope with chronic, acute or terminal illness through education, support/counseling and referrals for community resources. * In addition to conducting community visits, Social Workers may complete consultation for patient's inpatient at Lakeview Hospital to provide education about homecare, palliative care, and hospice. Work Schedule: 1.0 FTE; M-F 8:00am - 4:30pm Required Qualifications: Master's degree in social work from a CSWE accredited program
Must have Minnesota (MN) or Wisconsin (WI) Social Work license or license eligible upon start date: MN must have one of the following:
Licensed Graduate Social Worker (LGSW) or higher licensure. If LGSW, must be actively working on obtaining MN licensure at the LISW or LICSW level.
WI must have one of the following:
Certified Advanced Practice Social Worker (CAPSW) or higher license level. If licensed in only one state upon hire, must be willing to obtain license in the other state and provide proof of license within 4 months of start.
Must have vehicle, driver's license and current auto insurance. Must meet all criteria required under Lakeview's Automobile Liability Guidelines Strong psychosocial assessment skills and implementation of interventions based on social work assessment. Ability to function independently with minimal supervision. Experience and comfort providing services in the community, including patient homes. Knowledge of reimbursement procedures as well as knowledge of community resources. Experience and comfort with providing services in community settings, including patient homes. Ability to function on an interdisciplinary team. Ability to effectively communicate in written and verbal form within the interdisciplinary team. Ability to independently carry out directions and follow medical orders for services. Computer experience including documentation in an electronic medical record Comfort or experience working within serious illness or end of life. Ability to adapt to a flexible and changing schedule to meet patient needs. Willing to participate in On-Call rotating schedule
Preferred Qualifications:
