Position Summary


Renown Health and the University of Nevada, Reno School of Medicine (UNR Med) invite applications for an Advanced Practice Provider (APP) to join our growing High-Risk Breast Cancer Clinic within the William N. Pennington Cancer Institute. This full-time position serves as a primary clinical provider for patients at elevated risk for breast cancer, including those with hereditary cancer syndromes.


The APP will independently evaluate patients, perform comprehensive risk assessments, develop personalized surveillance and risk-reduction plans, prescribe preventive therapies, and collaborate closely with breast surgeons, medical oncologists, radiologists, pathologists, and genetic counselors. This role is integral to the academic-community mission of Renown and UNR Med, supporting clinical care, education, and program development. The position reports to the Medical Director of the Comprehensive Breast Program.


Responsibilities


Clinical Risk Assessment & Stratification




  • Conduct detailed personal and family history assessments related to breast and hereditary cancers.



  • Utilize validated breast cancer risk-assessment models (e.g., Tyrer-Cuzick, Gail, Claus, BOADICEA) to stratify patients into standard, high, and ultra-high risk categories.



  • Review and interpret prior breast imaging and pathology, including premalignant and high-risk lesions.





Genetic Risk Evaluation & Counseling




  • Provide pre-and post-test counseling for hereditary cancer genetic testing.



  • Identify patients who meet criteria for genetic testing for breast and other hereditary cancer syndromes.



  • Interpret genetic test results, including pathogenic variants, VUS, and negative findings.



  • Educate patients on cancer risks, screening recommendations, cascade testing, and risk-reduction strategies.



  • Coordinate referrals to specialized genetic counseling services when appropriate.





Risk Reduction & Medical Management




  • Develop individualized breast cancer risk-reduction plans.



  • Prescribe and manage endocrine risk-reducing therapies (tamoxifen, raloxifene, aromatase inhibitors) in alignment with clinical guidelines.



  • Monitor medication tolerance, side effects, and adherence.



  • Counsel patients on lifestyle and non-pharmacologic risk-reduction strategies.





Breast Pathology & Surveillance




  • Interpret and counsel patients regarding high-risk and premalignant breast lesions, including:




    • Atypical ductal hyperplasia (ADH)



    • Atypical lobular hyperplasia (ALH)



    • Lobular carcinoma in situ (LCIS)



    • Flat epithelial atypia and other high-risk findings





  • Recommend appropriate imaging surveillance (mammography, breast MRI) based on risk stratification.



  • Coordinate follow-up care and referrals to breast surgery or medical oncology as indicated.





Collaboration, Education & Program Development




  • Work collaboratively within a multidisciplinary breast care team.



  • Serve as a clinical resource for high-risk breast cancer management within the breast center.



  • Participate in quality improvement initiatives, clinical guideline development, and patient education efforts.



  • Maintain accurate and timely documentation in the electronic medical record.



  • Support the academic mission of UNR Med through teaching, mentorship, and participation in educational activities as appropriate.





Qualifications


Required




  • Current licensure as a Nurse Practitioner or Physician Assistant.



  • Graduate of an accredited NP or PA program.



  • National board certification (AANP, ANCC, NCCPA, or equivalent).



  • Eligibility for Nevada state licensure and DEA registration.



  • Demonstrated clinical experience in women's health, oncology, breast health, genetics, or a related specialty.



  • Strong communication skills and commitment to patient-centered care.





Preferred




  • Experience in breast oncology, hereditary cancer syndromes, or high-risk breast management.



  • Prior experience in an academic or multidisciplinary clinical environment.



  • Familiarity with breast imaging, pathology, and risk-assessment models.
