Ambulatory Nurse - Lung Center

GENERAL SUMMARY/ OVERVIEW STATEMENT:
*The Division of Pulmonary-Lung Center registered nurse is a professional who is responsible and accountable to provide care for patients in accordance with Nursing Standards of Care as guided by organizational policies. The RN should be registered in Massachusetts.
*Lung Center patients are a complex patient population and, as such, the registered nurse must possess a very strong clinical background. This nurse must be very well organized, have good attention to detail, demonstrate the ability to coordinate and follow through on all aspects of the patient's care, and effectively communicate with all members of the care team, the patient and the family.
*Demonstrates initiative, knowledge and clinical skills in caring for the patient with complex needs. Demonstrates the ability to effectively manage patients by assuming full responsibility for assessment, planning, implementation and evaluation of patient care.
*This RN is directly responsible to the Lung Center Practice Manager, Medical Director and Nursing Director for Medical Specialties.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
*Provide nursing support and care to patients within the Division of Pulmonary.
*Ensure that ambulatory patients are seen in a timely manner by the appropriate clinician(s) and that appropriate coordination of care and communication is maintained with the patients and referring providers.
*Perform clinical telephone triage, especially for urgent clinical situations, and general nursing support for patients across the Division of Pulmonary.
*Receive and address testing and laboratory results under specific protocol guidance and/or in collaboration with providers.
*Develop and implement patient education plans for the management of chronic lung disease.
*Maintain familiarity with department activity and a close working relationship with clinicians and support staff to provide the most appropriate triage and facilitate timely and effective communication and scheduling.
*Participate in departmental and organizational quality management and/or quality improvement projects as directed by the leadership team.
*Utilizes the electronic medical record and other information technologies in the care of patients.
*Communicates using SBAR format demonstrating evidence of the application of the nursing process.
*Working collaboratively to provide excellent care to patients and families in the safest environment.
*Other duties as assigned by the supervising physician or leadership team.
QUALITY OF CARE:
*Participates in multidisciplinary patient care conferences and medical management meetings.
*Observes principles of infection control in nursing practice and demonstrating awareness of potential/actual risks of infection and modes of transmission.
*Participates in all QAPI meetings and processes as assigned and appropriate.
*Develops a comprehensive plan of care based on data from an initial assessment of patient and family, information from other members of the health care team, intra-agency referral and previous medical records.
*Develops a comprehensive educational plan for the patient and family, utilizing appropriate resources and documents according to the department's documentation standards.
*Evaluates the effectiveness of the plan of care, and documents progress in meeting stated goals. Revises plan of care as needed to achieve desired outcomes.
*Provides a safe environment for patients, staff, family and visitors.
*Administers medication safely according to established policies and procedures.

*Regularly incorporates new scientific findings in specific area of expertise into clinical practice.

PARTICIPATION IN THE DEPARTMENT:
*Utilizes time and materials effectively.
*Participates in the orientation of new staff members as appropriate.
*Adheres to hospital and departmental policies. Recommends changes in policies and procedures through appropriate channels.
*Participates in determining and implementing goals and objectives for the Division of Pulmonary.
*Participates in routine group discussions to assist in process improvement.
*Please note that there will be no on-call responsibilities.

QUALIFICATIONS:

Education:

  • Graduate of an approved school of nursing with current registration in Massachusetts. Bachelor of Science Degree in Nursing is required.

Experience:

  • 1. Minimum of 2 years of recent (within 12 months) clinical nursing experience in an ambulatory clinic / practice setting required. Emergency room and/or procedural setting only not sufficient.

  • 2. Experience with telephone triage and telephone-based clinical assessment techniques of adult patient required.This is the safe, effective, and appropriate disposition of health-related problems and includes: assessing severity of symptoms, communicating urgent patient needs to the LIP, referral to appropriate setting or level of care, documenting interventions and an individualized plan of care using an EMR, education of patients and families on their disease process and interventions to promote health and well-being, preliminary interpretation of data, providing home health advice and follow-up, ability to make triage decisions quickly and calmly in emergency situations, and assuring continuity of care.

  • 3. Minimum of 2 years recent (within 1 year) experience with Pulmonary and/or Thoracic related patient care delivery as an RN, strongly preferred.

  • 4. Case Management experience preferred.

  • 5. Experience with team-based care preferred.

SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:

Show evidence of the basic analytic thinking necessary to care for a group of patients. Demonstrate strong interpersonal skills; must work closely and cooperatively with physicians and other health care professionals. Demonstrate the ability to set priorities. Function independently and well under stress. Must have excellent communication skills and clinical judgment and must have excellent interpersonal skills and flexibility. Must be able to handle multiple projects and re-prioritize as necessary and should possess the ability to perceive sensitive family issues and discuss these issues tactfully.

WORKING CONDITIONS:

Works in a busy and stressful patient care environment and in an office environment. May be exposed to communicable diseases and hazardous materials. Must work well independently and in a multidisciplinary group, and exercise flexibility.

Works within legal, regulatory, accreditation and ethical practice standards relevant to the position and as established by BWH/Partners; follows safe practices required for the position; complies with appropriate BWH and Partners policies and procedures; fulfills any training required by BWH and/or Partners, as appropriate; brings potential matters of non-compliance to the attention of the supervisor or other appropriate hospital staff.
