Transition of Care-Clinical Pharmacist, Population Health

Key Responsibilities:
-Conduct post-discharge medication reconciliations, ensuring accuracy, safety, and adherence to prescribed therapies.
-Educate patients and caregivers on medication management, including proper use, potential side effects, and drug interactions, utilizing motivational interviewing to encourage positive behavior change.
-Address medication affordability challenges, streamline therapy regimens, and connect patients with prescription assistance programs.
Collaborate with multidisciplinary teams to develop and implement personalized medication therapy plans, improve adherence, and enhance patient outcomes.
-Identify and mitigate potential drug-related medical problems to reduce hospital readmissions.
-Use telehealth technologies to engage patients remotely, ensuring seamless transitions of care while maintaining high-quality interactions.
-Document patient interactions and communicate recommendations to providers through the electronic medical record system.
-Participate in quality improvement initiatives and contribute to the development of best practices for transitions of care.

  • PharmD degree from an accredited institution and licensure to practice pharmacy in Massachusetts. Board certification and/or PGY1 residency are required.

  • At least 3 years in clinical pharmacy, with a focus on transitions of care or ambulatory settings preferred.

  • Exceptional clinical knowledge, communication abilities, and organizational skills. Proficiency in Microsoft Office and adaptability in a fast-paced environment are essential.
