The Wound/Ostomy Nurse is a Registered Nurse with advanced education, training and clinical expertise in the care of patients with wounds, ostomies and incontinence. The Wound/Ostomy Nurse serves as a clinical resource and consultant for all members of the healthcare team. The Wound/Ostomy Nurse collaborates closely with other Nurses as well as Nurse Practitioners and Providers. The Wound/Ostomy Nurse works with many specialties within the Multispecialty Clinic, including Surgical Wound Care, Acute Care Services, Plastics, Colon and Rectal and General Surgery. This role is responsible for developing, implementing, and evaluating the care, education, and rehabilitation of all patients with specific disorders of the gastrointestinal, genitourinary, and integumentary systems. The focus of this role is on the following key areas: * Assesses patient's needs related to wound/ostomy and continence issues incorporating functional, emotional, age-related and cultural information as indicated. * Derives the diagnosis and identifies healthcare issues based onthe assessment. * Provides education for the patient and the family on issues pertaining to wound/ostomy /continence and other health-related issues. * Works closely with Nursing, Medical and other staff to ensure maximal outcomes for patients with wound/ostomy or continence problems. - Develops and implements policies, procedures, and standards of care that guide and support the provision of evidence-based nursing practice and patient care. - Conducts assessments and develops educational programs for all levels of personnel to meet the unique needs of patients with wound, ostomy or continence problems. - Coaches and mentors nursing staff to manage the care for patients with wound, ostomy & continence problems. - Leads the Pressure Injury Prevention Program. - Collaborates with the Wound/Ostomy team members to lead regular pressure injury prevalence surveys. - Coordinates with the outpatient wound/ostomy service and utilizes the electronic medical record (EMR) to communicate and document the patient's care and ensure a seamless transition of care from inpatient to outpatient services. - Participates in quality and performance improvement initiatives to identify Wound/Ostomy Continence related issues. * Collects data, analyzes findings and formulates recommendations to improve patient care outcomes and excellence in nursing practice for every patient.

The ideal candidate will have the following experience: Experience: * Minimum of two years of Nursing experience * Prior wound/ostomy experience required. --- General Care or Medical Surgical nursing experience preferred. Licenses, Certifications and/or Registrations: * Current licensure as a Registered Nurse in the Commonwealth of Massachusetts. * Current certification as a Certified Wound Ostomy Nurse (CWON) or Wound, Ostomy, Continence Nurse (CWOCN, WOCNCB) required.
