Job Overview

The Dialysis Nurse Entry supports Children's Hospital Colorado (CHCO) mission, vision, and core values. This role provides direct patient care for patients experiencing acute or chronic kidney failure in accordance with applicable scope and standards of practice and policies, values, and mission of the organization. The Dialysis Nurse Entry monitors and manages patients during acute and chronic dialysis treatments to maintain a safe environment, optimize outcomes, and maximize available resources.

Shift: The shift is 0630-1900 rotating days, Monday through Saturday. We have rotating weekend requirements. *On call component to be discussed.

Qualifications

Education: Bachelor of Science in Nursing (BSN)

Experience: One year nursing experience

Certification(s): BLS/CPR from the American Heart Association with at least 6 months left before expiration is required upon hire.

Licensure(s): Registered Nurse (RN) license

POPULATION SPECIFIC CARE

Neonate - <30 days

Infancy - >30 days to 1yr

Toddlers - >1yr to 3yrs

Pre-Schoolers - >3yrs to 5yrs

School age - >5yrs to 13yrs

Adolescent - >13yrs to 18yrs

Adult - >18yrs to 65yrs

ESSENTIAL FUNCTIONS

An employee in this position may be called upon to do any or all the following essential functions. These examples do not include all the functions which the employee may be expected to perform.

  1. Responsible for the oversight and delivery of direct patient care aligning with established policies, procedures, and state and federal regulations.
  2. Direct patient care includes all dialysis modalities: hemodialysis (HD), peritoneal dialysis (PD), Acute HD and PD. Demonstrates knowledge of Transplant along with other therapies as needed.
  3. Implements prescribed treatments including Sodium (Na) modeling and Ultrafiltration modeling (when appropriate), ensuring stable treatment therapy.
  4. Assesses patient response to treatment, communicates changes to Nephrology Providers and takes appropriate intervention; actively participates in the pre-evaluation, initiation, monitoring, termination, access care, and post evaluation of patients receiving treatment according to established procedure.
  5. Assesses patient and family readiness and ability to perform dialysis including patient's home environment if necessary. Directs patients in the preventative maintenance of home therapies equipment, including water systems and analyses as appropriate.
  6. Develops and maintains collaborative professional relationships with interdisciplinary healthcare team to provide culturally competent, age-appropriate patient/family care.
  7. Evaluate and adjust care as needed; accountable for responding to and resolving barriers with providers, patients, family members, and hospital personnel in a timely manner.
  8. Oversees and maintains the patient records including both hardcopies and electronic, for accuracy.
  9. Collaborates with patients, families, providers, and a multidisciplinary team to facilitate family meetings and as needed to establish a mutually developed plan of care and in an environment that fosters shared decision making.
  10. Assess patient's and family's SDoH (Social Determinants of Health) at home or in other community locations and collaborate with social work and family navigators as needed.
  11. Provides education and coaching to patients and families to improve health literacy and work towards self-management of their health; ensures education needs of patients and families are being met.
  12. Facilitates coordination of patient access to appropriate medical and specialty providers, medication acquisition and management, procedures/tests, appointments, and referrals.
  13. Participates in supporting the learning environment for team members for are precepting, orienting and learning.
  14. On Call responsibilities for the Acute Dialysis setting, nights and weekends as scheduled

Supervision Received: Under close supervision, the employee receives training to develop skills and abilities in a specific line of work or general occupational area. Work product is subject to close, continuous inspection.

Salary Information

Annual Salary Range (Based on 40 hours worked per week): $72,340.07 to $108,510.10
Hourly Salary Range: $34.78 to $52.17
