We are actively seeking a highly motivated, innovative and experienced leader to join our team as the Community Health Outreach Worker. Compensation will be commensurate with experience. Position Summary: The Community Health Outreach Worker (CHOW) is a part-time position that assists with the outreach, education and counseling of a specified group of members; consistent with the philosophy, goals and objectives of Amida Care. Assists clients in adhering to their individual disease processes and care management plans. Incumbent may be a Plan member of Amida Care. Responsibilities:
